Adding Incidents for a Category

This guide explains how to add an incident and create a new Supplemental Incident Report (SIR) within an operation category.

Adding a SIR

Adding a SIR After selecting an operation category, click the + Add MTO, Add RWC, Add LTI, or Add FTL button (depending on the type of the incident) to add an incident and create a new SIR.

The quantity of incident reports for each type is indicated by the number shown next to it.
The SIR number is a unique identifier — it has no significance beyond distinguishing individual SIRs.
To quickly view the answers to an SIR, click Show Answers. For a new SIR, all questions will be unanswered by default. New SIR

Editing a SIR

Editing a SIR To begin completing the required answers for a Supplemental Incident Report, simply click on the Edit button of the desired SIR.

Optional and required SIR information

Optional and required SIR information The SIR form has optional and required questions that can be completed. A SIR is considered incomplete until all required questions have been answered. Optional SIR Information: These answers are for the participant company’s use and records and may be used at a later date for advanced reporting.
  • Date of Work
  • Rig Name/Number
  • Is this a SIF (Serious Injury or Fatality)
  • SIF Comments (if applicable)
Required SIR Information: These are the standard questions that have been historically required for a SIR. An answer must be provided for each of these questions for an SIR to be considered COMPLETE.
  • Occupation Type
  • Body Parts
  • Incident Type
  • Equipment Type
  • Operation Type
  • Location
  • Time In Service for Company
  • Time of Day
  • Age of Injured
For each question there is an option for Not Reported and Other, if the available answers do not apply.

Answering the Required SIR Questions

Answering the Required SIR Questions For each required question, only 1 answer can be selected. When an answer is selected, you will automatically move to the next required question. You may navigate between questions using the Next and Previous buttons above the question. You may also go back to the SIR Summary page (to review your answers) at any time by clicking the Go to Summary Page link above the question.

Save Changes

Save Changes After you have finished answering all of the required SIR questions, click Save Changes to complete this supplemental incident report. At any point while editing an SIR, the changes made can be discarded or saved.
Discarded changes can not be recovered. If you click Discard Changes, you will be asked to confirm this decision. This will discard any changes you have made to optional or required questions while the edit SIR window was open.
